EMT Chapter 26-MCIT-F3-2220 | The Dyspnea Divide

EMT Chapter 26-MCIT-F3-2220 | The Dyspnea Divide: Not all breathing emergencies are alike. Some patients struggle because oxygen is not reaching the body and some struggle because air cannot move through the airways. Both feel like breathlessness , yet each demands a different assessment approach. The course uses a two track framework – the chemical -perfusion track and the mechanical -ventilation track . It also covers metabolic and hyperventilation conditions that mimic true respiratory emergencies.

 The lessons covered are

  • Lesson 1:  The Neurobiology Of Dyspnea – Chemistry vs. Mechanics
  • Lesson 2:  Deceptive “Shortness of Breath” – The Chemical-Perfusion Track
  • Lesson 3:  Overt “Difficulty Breathing” – The Mechanical-Ventilation Track
  • Lesson 4:  Sorting Out the Hyperventilation & Metabolic Traps, and,
  • Lesson 5:  Target-Specific BLS Management & Medical Handoffs

EMT Chapter 26-MCIT-F3-2220 | The Dyspnea Divide

Pull the chemical track together into one clear picture. The chemical track is fundamentally a problem of content or delivery. Either the right substances are not in the blood, or the blood is not getting where it needs to go. The oxygen is not loading properly, carbon dioxide is building up, or circulation is failing to carry the supply to the tissues. The lungs, as a set of bellows, may be working perfectly well. Because the failure is internal and chemical, this track is often quiet on the outside. The patient may be sitting still. The breathing may not look dramatic. And this is exactly where the pulse oximeter can lull a provider into a false sense of security, because saturation readings can stay normal in several of these conditions right up until the patient deteriorates.

The conditions that live on this track share that theme. Heart problems, blood clots in the lungs, carbon monoxide poisoning, anemia, and the early stage of fluid in the lungs. In everyone, the complaint is real and often severe, but the body is not putting up a visible fight. This is the track that maps onto what the course calls air hunger. The next lesson takes this track apart condition by condition, but the mental model to carry forward is simple. When breathlessness is severe, and the body looks calm, suspect the chemistry. Suspect delivery. And do not let a comfortable-looking patient or a comfortable-looking number talk you out of your concern.

Core Concepts & Takeaways

  • Differentiate the chemical-perfusion origin of dyspnea (air hunger) from the mechanical/ventilation origin (increased work of breathing) using the sensory pathways that generate each sensation.
  • Identify the deceptive chemical-perfusion presentations of acute pulmonary edema, pulmonary embolism, carbon monoxide poisoning, and anemia, including the limitations of pulse oximetry in each.
  • Differentiate lower airway from upper airway mechanical obstruction using presentation and adventitious sounds, and describe the exhaustion timeline that precedes respiratory failure.
  • Differentiate psychogenic hyperventilation from compensatory hyperventilation, and apply objective assessment findings before attempting to slow a patient’s breathing.
  • Select target-specific BLS management within the EMT scope for each track, and formulate a track-aware handoff report that activates appropriate receiving resources.
