EMT Chapter 26-MCIT-F3-2218 | Navigating Peculiar EMS Calls

EMT Chapter 26-MCIT-F3-2218 | Navigating Peculiar EMS Calls

This EMT level continuing education  course is designed to prepare providers for calls that do not look like text book scenarios. Over five lessons it covers five genuinely unusual call environments , where standard training gets the crew to the door and , and real world adaptability gets everyone through it

The lessons covered are

  • Lesson 1:  The Labyrinth — Hoarding & Environmental Barriers
  • Lesson 2:  The Human Anchor: Crush & Entrapment
  • Lesson 3:  The Center Ring: Sensory Overload & Mass Gatherings
  • Lesson 4:  The Indoor Jungle: Exotic Pets & Zoonotic Risks, and,
  • Lesson 5:  The Industrial Maw: Complex Machinery Entrapment

The tourniquet in crush syndrome management serves a purpose that is fundamentally different from its role in hemorrhage control — and understanding that difference is what makes the decision clinically defensible.

In hemorrhage control, a tourniquet stops arterial blood from reaching a wound. In crush syndrome management, the tourniquet is applied to the trapped extremity before the compressive force is removed. Its purpose is to gate the return of the toxic load to systemic circulation. By maintaining compression on the venous outflow of the extremity at the moment the mechanical compression is released, the potassium-rich, acidotic, myoglobin-laden blood is prevented from flooding the general circulation all at once.

This buys time. Time for the ALS provider to complete medical preparation. Time for the cardiovascular system to begin adapting to what will eventually enter the circulation. Time for the clinical situation to be as controlled as it can possibly be at the moment of greatest physiological risk.

Core Concepts & Takeaways

  • Identify the environmental and clinical hazards specific to hoarding scenes, and describe the respiratory, septic, dermatologic, and self-neglect findings that result from chronic environmental exposure.
  • Describe the pathophysiology of crush syndrome, recognize the clinical and ECG warning signs of hyperkalemia, and apply EMT-scope interventions that prepare the entrapped patient for the moment of release.
  • Demonstrate a complete trauma assessment adapted to a high-noise mass-gathering environment, analyze the injury patterns produced by multi-stage deceleration mechanisms, and formulate the transport decision and hospital notification.
  • Differentiate anaphylaxis from direct envenomation, apply the indications and administration parameters for epinephrine within EMT scope, and manage the airway of the neurotoxically envenomated patient.
  • Manage a prolonged industrial machinery entrapment by verifying lock-out/tag-out, analyzing the Lethal Triad in a warm environment, applying prolonged field care monitoring, and evaluating indications for a Go-Team request.
