EMT Chapter 26-MCIT-F3-2217 | The Art Of Presence: End Of Life Care In EMS :

The  End-of-calls challenge EMS providers in ways that standard clinical frameworks do not fully address. They require managing uncertainty, navigating family dynamics, holding ethical boundaries, and operating in situations where comfort , clarity and composure matter as much as intervention. The Goal here is not to replace medical training but to add a layer that makes the training much more effective in such situations.

The lessons covered are

  • Lesson 1:  The Changing EMS Landscape
  • Lesson 2:  Understanding Palliative and Hospice Care
  • Lesson 3:  Legal and Ethical Framework
  • Lesson 4:  The Art of Presence: Skills Beyond the Protocol, and,
  • Lesson 5:  Provider Wellness and Moral Injury

Excerpt from EMT Chapter 26-MCIT-F3-2217 | The Art Of Presence: End Of Life Care In EMS

Advance directives occupy a different legal space from MOLST and DNR orders, and that difference matters in the field.

An advance directive — whether it is a living will or a healthcare proxy designation — is a legal document created by the patient, usually with an attorney or through a state-provided form. It reflects the patient’s expressed wishes for their own care. It is a meaningful document. But it is not a physician’s order.

In prehospital settings, EMS operates on physician orders. A MOLST and a DNR are both signed by a physician. An advance directive is not. What this means practically: if the only document present is a living will, EMTs cannot treat it as a MOLST. They can use it as guidance — particularly if it is consistent with the clinical picture — but it does not carry the same immediate operational weight.

The healthcare proxy is a different matter. If a patient lacks decision-making capacity and a designated healthcare proxy is present, that person has legal authority to make medical decisions on the patient’s behalf. That authority is meaningful and should be respected. But it still cannot override a valid physician’s order. If a MOLST says Comfort Measures Only and the healthcare proxy is demanding aggressive intervention, the MOLST takes precedence. The proxy’s authority exists in the absence of a physician’s order — not in competition with one. These distinctions matter because they are exactly what gets sorted out in the first minutes on scene.

Core Concepts & Takeaways

  • Describe the structure and function of palliative and hospice care systems, including the role of the hospice nurse, the comfort kit, and the EMT’s scope of practice within these settings.
  • Differentiate between the legal documents that govern end-of-life care, including DNR orders, MOLST and POLST forms, and advance directives, and identify their appropriate application in prehospital settings.
  • Apply evidence-based communication and presence techniques to manage end-of-life scenes, including positioning, environmental management, and family interaction strategies.
  • Analyze complex end-of-life scenarios involving family disagreement, missing or contradictory documentation, and the 2025 Connecticut “Treat in Place” protocol to formulate appropriate clinical and ethical responses.
  • Evaluate the signs of moral injury and helper’s guilt in EMS providers and formulate strategies for self-care, peer support, and resource utilization to promote long-term provider wellness.
