Let’s talk about how anatomy affects axis because this helps you understand why different patients have different normal axes. The physical position of the heart in the chest directly influences the electrical axis direction.Think about a tall, thin person—their chest cavity is long and narrow. The heart tends to “hang” more vertically in that narrow space, so the axis tends to be more vertical, closer to +90 degrees. Contrast that with a shorter, stockier person—their chest is proportionally wider. The heart sits more horizontally, so the axis tends to be more horizontal.Pregnancy is a perfect example of physiologic axis change. As the baby grows, it pushes the diaphragm upward, which pushes the heart upward and more horizontal. The axis shifts leftward. This is completely normal for pregnancy—it’s not pathology, it’s anatomy adapting to circumstances.Same thing with obesity—elevated diaphragm from abdominal content pushes the heart into a different position. With COPD and hyperinflated lungs, the diaphragm gets flattened and the heart’s position changes, often causing right axis deviation.
These are physiologic variations. Understanding them helps you recognize what’s normal variation versus what’s pathologic deviation.
