Excerpt from EMS Care EMT Chapter 24-MCIT-F3-2150 | Culture Of Safety
Error prevention is about creating habits that catch mistakes before they happen. Use the ‘rule of three’—check critical interventions three times. Cross-verify important information with your partner. Take safety pauses during complex procedures—these brief stops let you verify that you’re on the right track. Use clear, specific communication—repeat important information. Follow standard procedures every time—they exist because they work. These aren’t time-wasters; they’re safety nets that protect you and your patients.
Just culture recognizes the difference between honest mistakes and reckless behaviour. If you make a mistake while following protocols and trying your best, that’s different from deliberately ignoring safety procedures. Just culture encourages reporting because we learn from mistakes – both our own and others. It balances accountability with support. When errors happen, the focus is on ‘what went wrong’ not ‘who messed up.’ This approach creates an environment where people feel safe reporting issues, and everyone learns from experience.
Core Concepts & Takeaways
- Demonstrate the ability to evaluate a simulated emergency scene using a structured risk assessment approach, identifying at least three potential hazards and appropriate mitigation strategies.
- Develop and present a personal safety plan incorporating vehicle operations, equipment handling, and environmental hazard management strategies.
- Analyze an EMS error case study, apply just culture principles to determine system versus individual factors, and recommend prevention strategies.
- Practice and evaluate team-based safety communication techniques during a simulated high-stress scenario, demonstrating closed-loop communication and situational awareness.
- Create a fatigue management and mental health wellness plan tailored to the unique challenges of EMS work schedules, including specific strategies for recognizing and addressing stress indicators.
