Let’s walk through your decision-making process. When you arrive on the scene, you’re faced with an unconscious patient. Your initial assessment reveals slow breathing at 6 per minute, pinpoint pupils, and cyanotic lips. Drug paraphernalia is present.
This is your decision tree:
First, ensure scene safety and BSI.
Second, begin respiratory support with BVM while your partner prepares naloxone.
Third, determine if one provider can maintain adequate ventilation. If not, prioritize BVM ventilation with two providers over immediate naloxone administration.
Every decision you make follows from your assessment findings and works toward improving respiratory status.
