Cognitive assessment is critical in geriatric care, but can be challenging. Start by determining their baseline mental status – is confusion new or chronic? Acute changes are more concerning than long-standing conditions. When assessing orientation, remember that some elderly patients may not know the date but can tell you about recent events.

Watch for red flags that suggest acute problems. Sudden confusion, personality changes, or new behaviors often indicate serious conditions like infection, stroke, or medication issues. What family members tell you about recent changes is often more valuable than a single assessment.

Document thoroughly but objectively. Include specific behaviors and changes noted by family or caregivers. Remember that poor medication compliance often contributes to mental status changes, so review their medication history carefully.
