Excerpt from EMS Care EMT Chapter 24-MCIT-F3-2117 | Neurological Emergencies
EMT management of stroke patients focuses on rapid assessment and supportive care. Use the stroke scales we discussed earlier for quick assessment. Manage ABCs as always. Administer oxygen to maintain Saturation of peripheral oxygen above 94%. If within your scope of practice, establish I V access. Always check blood glucose, as hypoglycemia can mimic stroke symptoms. Keep the patient NPO or nothing by mouth in case emergency surgery is needed. Position the patient with their head elevated about 30 degrees to help reduce intracranial pressure. Reassess the patient frequently, as stroke symptoms can evolve or resolve rapidly. Finally, rapid transport should be provided to an appropriate stroke-capable facility.
Core Concepts & Takeaways
- Demonstrate proficiency in performing comprehensive neurological assessments using standardized tools, including the Glasgow Coma Scale, FAST exam, and pupillary assessment.
- Accurately identify and differentiate between types of strokes (ischemic and hemorrhagic), implementing appropriate emergency care protocols within the critical time window.
- Recognize and respond appropriately to various seizure types and status epilepticus, including proper patient care during and after seizure activity.
- Assess and manage patients with altered mental status using the A E I O U-TIPS mnemonic while demonstrating an understanding of various causes and appropriate interventions.
- Evaluate and implement appropriate care for head and spinal cord injuries, including proper spinal motion restriction techniques and concussion assessment.
