Paramedic Recert EMS Care | 23-MCIT-F3-4101 | Ventilation

Paramedic Recert EMS Care | 23-MCIT-F3-4101 | Ventilation starts with the introduction to the topic. It outlines the coverage and objectives.. The course takes the student through details of the Anatomy of the respiratory system, followed by discussions on Lung capacity and volume. The course dives into methods of assessing the breathing,  and airway. Further the lessons discus on managing the airway under different contexts. Oropharyngeal Airways and nasopharyngeal are also covered with details on Adjuncts. The different airway procedures like subglottic and supraglottic  are dealt with followed by Artificial ventilation. The lessons are

The lessons covered are :

  • Lesson 1:  Lung Volumes and Capacities
  • Lesson 2:  Recognizing Adequate Breathing, Respiration and Ventilation
  • Lesson 3:  Airway Assessment
  • Lesson 4:  Advanced Airway Management
  • Lesson 5:  Basic Airway Adjuncts
  • Lesson 6:  Advanced Airway Procedures
    Lesson 7:  Augmenting Patient Ventilation

Core Concepts & Takeaways

  • Differentiate between adequate and inadequate breathing.
  • Differentiate between respiratory distress and respiratory failure.
  • Differentiate between alveolar ventilation and minute ventilation.
  • Explain when to oxygenate and when to ventilate a patient.
  • Examine the effect of ventilation on venous return and cardiac output in a spontaneously breathing patient and an Artificially ventilated patient.
  • Identify the use of automated transport ventilators when managing patients.
  • Discuss the AHA’s position on routine suctioning of the newborn. And,
  • Assess the advantages and disadvantages of various advanced airway adjuncts.

An Excerpt from the course on Ventilation

Airway management algorithms provide a preplanned approach to airway management.  They ensure providing the best patient care possible to a patient who initially cannot be ventilated or whose airway cannot be secured.  An algorithm provides a specific planned set of actions and decisions where the “if this––then that” decisions are made in advance.  This prevents inaction, wrong action, and panic.

In every patient encounter, a paramedic applies a group of patient care activities based on a presumptive conclusion or working diagnosis.  This is mostly based on a previously planned and documented protocol.  Apart from some specific order details, the actual order in which they are carried out does not affect the patient outcomes. However, the Paramedic will perform all the care specified in the protocol.

In certain emergencies (e.g., cardiac arrest with ventricular fibrillation), specific tasks must be carried out the same way, in the same order, in a minimal time period. Furthermore, the care must be modified based on the patient’s response to the interventions. In these cases, algorithms are applied, primarily as “if-then” branch points.

Although the use of algorithms can greatly facilitate airway management, it is important to remember that they are written for the majority of situations and that algorithms are not “one size fits all.
