So what exactly is a stroke chameleon? Think of it as a stroke that comes to the party wearing a disguise. These are the strokes that present with symptoms so subtle, so unusual, or so seemingly unrelated that they’re mistaken for everything from vertigo to psychiatric conditions. A chameleon stroke might show up as isolated dizziness that looks exactly like benign positional vertigo. It might present as sudden confusion that the family assumes is a UTI in grandma. Or it could be that 35-year-old with the worst headache of their life who everyone assumes just has a migraine. Studies show that up to 30% of strokes are initially missed or misdiagnosed, and many of these are our chameleons. They’re particularly common in younger patients – because we don’t expect strokes in 30-year-olds – and in posterior circulation strokes that affect the back of the brain. These posterior strokes are notorious troublemakers because they often spare the motor cortex, meaning your patient might have full strength and clear speech while their cerebellum or brainstem is under attack. The challenge for us as EMTs is that we need to maintain that high index of suspicion without becoming the providers who cry wolf on every dizzy patient.
